ABSTRACT

AIM: To compare the short-term postoperative visual acuity, visual disturbance phenomena, and spectacle independence rate in patients who underwent monocular implantation with trifocal, multifocal, or extended range of vision intraocular lens(IOL).METHODS: A retrospective analysis was conducted on 67 cataract patients(67 eyes)who underwent phacoemulsification cataract extraction combined with IOL implantation from March 2019 to December 2022. A total of 35 cases(35 eyes)received Symfony extended range of vision IOL implantation, 21 cases(21 eyes)received AcrySof IQ ReSTOR +3D multifocal IOL, and 11 cases(11 eyes)received AcrySof IQ PanOptix trifocal IOL. The preoperative uncorrected distance visual acuity(UDVA), uncorrected intermediate visual acuity(UIVA), and uncorrected near visual acuity(UNVA)and 3 mo postoperatively were documented. Moreover, defocus curves, visual disturbance phenomena, and spectacle independence rates were recorded at 3 mo postoperatively.RESULTS: At 3 mo postoperatively, no statistically significant differences were observed in UDVA among the three groups(P>0.05). A comparison of UIVA showed superior results in the Symfony and PanOptix groups compared to the ReSTOR group(all P<0.01). The UNVA of both the ReSTOR and PanOptix groups outperformed the Symfony group(all P<0.01). The defocus curves indicated that in the intermediate vision range(-1.00 to -1.50 D), the Symfony group exhibited better performance than the ReSTOR group(P<0.05); while in the near vision range(-2.50 to -3.50 D), the ReSTOR group was superior to the Symfony group(P<0.05). The PanOptix group demonstrated superior visual acuity in the near vision range(-2.00 to -3.50 D)compared to the Symfony group(P<0.05)and was also superior in the intermediate vision range(-1.00 to -2.00 D)compared to the ReSTOR group(P<0.05). No significant differences were observed in the incidence of glare or halo and binocular interference phenomena among the three groups(P>0.05). The PanOptix and ReSTOR groups exhibited a higher spectacle independence rate compared to the Symfony group(P<0.0167).CONCLUSION: Compared to Symfony extended range of vision IOL and ReSTOR multifocal IOL, PanOptix trifocal IOL offers a balanced approach to distance, intermediate, and near visual acuity, without a high incidence of glare and halo, and with a higher spectacle independence rate. Caution is still advised when considering monocular implantation with presbyopia-correcting IOLs.

ABSTRACT

Objective:To investigate the effect of peripheral plus ring size and its potential associations with pupil diameter (PD) on axial length (AL) elongation in myopic patients after wearing orthokeratology lenses. Methods:A case-control study was conducted. One hundred eyes from 100 myopic patients who underwent orthokeratolokgy lenses fitting were enrolled at Beijing Ming Vision and Ophthalmology from January to June 2020.AL and central corneal thickness (CCT) measurements were obtained using Lenstar LS900 ocular biometry, and steep keratometry (Ks), flat keratometry (Kf) were assessed using the TMS-4 corneal topographer. The patients were divided into larger peripheral plus ring diameter (LPPRD) group consisting of 55 eyes with PD<PPRD-0.9 mm and smaller PPRD (SPPRD) group consisting of 45 eyes with PPRD-0.9 mm <PD<PPRD+ 0.9 mm based on PPRD and PD sizes in the tangential difference map.Differences in the above parameters before and after wearing orthokeratology lenses were compared between the two groups. The study followed the Declaration of Helsinki. The study protocol was approved by the Ethics Committee of Ineye Hospital of Chengdu University of Traditional Chinese Medicine (No.2021yh-008). All the subjects and their guardians understood the purpose and method of the study and signed the informed consent voluntarily. Results:The PPRD of LPPRD group was (5.15±0.37)mm 12 months after wearing orthokeratology lenses, which was significantly higher than (4.75±0.32)mm of SPPRD group ( t=0.214, P<0.01). The elongation of AL at 3, 6, 9 and 12 months after wearing orthokeratology lenses were (0.13±0.07), (0.23±0.11), (0.31±0.13) and (0.36±0.41)mm in LPPRD group, and (0.06±0.05), (0.11±0.06), (0.14±0.07) and (0.17±0.08)mm in SPPRD group, respectively. There were significant differences in AL elongation at different time points after wearing orthokeratology lenses between the two groups ( Fgroup=30.527, P<0.01; Ftime=16.528, P<0.01), and the AL elongation at different time points was smaller in SPPRD group than in LPPRD group, showing statistically significant differences (all at P<0.01). Compared to before wearing orthokeratology lenses, Ks and Kf were significantly smaller and CCT was thinner 3, 6, 9, and 12 months after wearing orthokeratology lenses in both groups (all at P<0.01). There was no significant difference in Ks, Kf, and CCT between the two groups (all at P>0.05). There was no significant difference in PD at different time points before and after wearing orthokeratology lenses between the two groups ( Fgroup=1.133, P=0.293; Ftime=231.427, P=0.112). Conclusions:The size of peripheral plus ring and its relationship with PD are key factors influencing the AL elongation rate in myopic patients after wearing orthokeratology lenses. With smaller PPRD, patients have slower AL elongation and better myopia control.

ABSTRACT

Objective:To observe the effects of blue light intervention on the development of optical defocus-induced myopia in guinea pigs and investigate its underlying mechanisms.Methods:Forty-eight normal-grade two-week-old tricolor guinea pigs were randomly divided into a blue light group and a white light group, with 24 animals in each group. The right eye of guinea pigs was fitted with a -5.00 D lens to establish an optical defocus model as the experimental eye, while the left eye served as the control without any covering. Before the experiment and after 8-week intervention, the refractive power of guinea pigs was measured by streak retinoscopy. The anterior chamber depth, lens thickness, and axial length were measured by A-scan ultrasonography.Corneal curvature radius was determined using a keratometer. After 8-week intervention, the guinea pigs were euthanized through overanesthesia, and the right eyeballs were enucleated and the retinas were isolated. The density of S and M cone cells of the guinea pig retinal sections were observed via immunofluorescence staining. The expression of retinal retinoic acid was assessed by high-performance liquid chromatography. The expressions of retinoic acid receptor (RAR-β) in the retina and matrix metalloproteinase-2 (MMP-2), tissue inhibitor of metalloproteinase-2 (TIMP-2), and type Ⅰ collagen in the sclera were detected by real-time fluorescence quantitative PCR.Changes in scleral thickness were observed through hematoxylin-eosin staining. The use and care of the animals complied with Regulations for the Administration of Affair Concerning Experimental Animals by State Science and Technology Commission. The study protocol was approved by the Ethics Committee of Eye and ENT Hospital of Fudan University (No.2022ETKLD10032).Results:After 8 weeks of intervention, guinea pigs in the blue light group showed (0.63±0.12)D of relative hyperopia and a deceleration of axial elongation by (0.08±0.00)mm compared with the white light group in the right eye.In the left eye, guinea pigs in the blue light group showed (0.42±0.09)D of relative hyperopia and a deceleration of axial elongation by (0.08±0.00)mm compared with the white light group. The guinea pigs in blue light group showed (1.52±0.09)D of myopia in the right eye compared with the left eye, with an increase in axial elongation of (0.06±0.00)mm.The guinea pigs in white light group showed (1.66±0.07)D of myopia in the right eye compared with the left eye, with an increase in axial elongation of (0.13±0.00)mm, and the differences were statistically significant (all at P<0.05). The density of M cone cells was lower and density of S cone cells was higher in the blue light group in the dorsal and ventral sides of the retinal sections compared with the white light group, showing statistically significant differences ( t=32.33, 52.23, 42.09, 25.02; all at P<0.05). The deceleration of myopia progression in the blue light group was strongly positively correlated with the increase in S cone cell density on the ventral side ( r=0.95, P<0.01). The expression levels of retinoic acid, RAR-β, and MMP-2 were decreased, and expression levels of TIMP-2 and type Ⅰ collagen were increased in blue light group compared with the white light group, showing statistically significant differences ( t=18.73, 7.45, 3.72, 6.19, 9.03; all at P<0.05). The scleral thickness in the blue light group was (125.0±7.8)μm, which was significantly thicker than (102.0±6.3)μm in the white light group ( t=26.93, P<0.05). Conclusions:Blue light intervention can inhibit the progression of defocus-induced myopia in guinea pigs.Refractive power changes in guinea pigs may be influenced by alterations in retinal cone cell density, retinoic acid expression, and scleral collagen expression.

ABSTRACT

AIM: To observe the effect of eccentricity of the optical treatment zone on retinal defocus after wearing orthokeratology lens for 12mo.METHODS: In this case-control study, a total of 120 myopic patients(120 eyes)who completed the fitting in our hospital from March 2021 to September 2021 and insisted on wearing orthokeratology lenses for 12mo were selected. According to the eccentricity after wearing lenses for 12mo, they were divided into the low eccentricity group(&#x003C;0.5mm, 58 cases, 58 eyes)and the moderate and high eccentricity group(≥0.5mm, 62 cases, 62 eyes). The optical treatment zone diameter(OTZD), eccentricity, axial length(AL), pupil diameter(PD)and refraction difference value(RDV)were evaluated after 12mo of wearing orthokeratology lenses, and the correlation between RDV and eccentricity was analyzed.RESULTS: After wearing orthokeratology lenses for 12mo, AL growth and RDV at 30°~40° and 40°~53° from the fovea were significantly lower in the moderate and high eccentricity group than in the low eccentricity group(all P&#x003C;0.05). In all patients, RDV at 40°~53° from the fovea was positively correlated with AL growth and OTZD(rs=0.544, 0.333, both P&#x003C;0.01), and negatively correlated with eccentricity(rs=-0.224, P=0.014).CONCLUSION: Peripheral retinal defocus is related to eccentricity and OTZD after wearing orthokeratology lenses. The greater eccentricity and the smaller OTZD within a certain range can induce more peripheral retinal myopic defocus, leading to a better control of AL growth.

ABSTRACT

The incidence of myopia is gradually on the rise worldwide, which seriously affects the eye health of teenagers and children, causing enormous loss of socioeconomic benefits. As a result, the prevention and control of myopia is crucial and urgent. In recent years, orthokeratology lens have gradually demonstrated its superiority in the field of myopia prevention and control. At present, the principle of controlling the development of myopia by orthokeratology lens is mainly based on the theory of retinal hyperopia optical defocus, which promotes the shift of hyperopic defocus to myopic defocus in myopic patients to curb the growth of the axial length. The effect of controlling the development of myopia is related to various factors, including the total amount of defocusing, pupil diameter, optical zone design, and lens decentration. The widespread use of orthokeratology lenses will effectively reduce the incidence of myopia in teenagers and children. This paper discusses the principle of controlling the development of myopia by the defocus technique of orthokeratology lenses, and the relationship between the amount of defocusing and the position of the defocusing circle and the effect of myopia prevention and control. A specific review was conducted to clarify the research progress on defocus technique of orthokeratology lens in the prevention and control of myopia.

ABSTRACT

AIM: To investigate the effect of peripheral defocus spectacles and orthokeratology lenses on the control of axial length in children and adolescents with myopia.METHODS: Prospective study. A total of 71 cases(134 eyes)of children and adolescents with myopia who visited the Second Hospital of Longyan from June 2019 to June 2021 were selected. They were fitted with peripheral defocus spectacles for 12mo and then switched to orthokeratology lenses. The growth of axial length was observed at 3, 6, and 12mo after wearing peripheral defocus spectacles and orthokeratology lenses.RESULTS: The median axial length growth after wearing peripheral defocus spectacles and orthokeratology lenses for 12mo was 0.35 and 0.14mm, respectively. The axial growth at 3, 6, and 12mo after wearing orthokeratology lenses was lower than those after wearing peripheral defocus spectacles(P&#x0026;#x003C;0.001), and the growth rate of axial length was significantly reduced. The patients were divided into a rapid progression group(axial growth ≥0.4 mm, 29 cases, 54 eyes)and a non-rapid progression group(axial growth &#x0026;#x003C;0.4mm, 42 cases, 80 eyes)according to the axial growth of peripheral defocus spectacles for 12mo. The median axial growth after wearing peripheral defocus spectacles for 12mo in the two groups was 0.70 and 0.24mm, respectively, while the median axial growth after wearing orthokeratology lenses was 0.31 and 0.09mm, respectively. The growth rate was reduced by 56% and 63% respectively in the two groups after wearing orthokeratology lens. The axial growth of cases wearing orthokeratology lenses for 12mo in the non-rapid progression group was lower than that in the rapid progression group, and it did not change with age or diopter. There was no significant difference among different ages and different diopters in the rapid progression group(P&#x0026;#x003E;0.05). In the non-rapid progression group, axial growth of cases aged 7-12 years was higher than those aged 13-16 years(P&#x0026;#x003C;0.05), but there was no significant difference among different diopters(P&#x0026;#x003E;0.05).CONCLUSION: Orthokeratology lens is more effective than peripheral defocus spectacles in controlling axial growth in children and adolescents with myopia, and the control effect of orthokeratology lens on rapid-progressing myopia is remarkable.

ABSTRACT

Purpose: To compare four different types of intra ocular lenses (IOLs), namely, AT LISA, Eyecryl SERT trifocal, Eyhance, Eyecryl SERT extended depth of focus (EDOF) with respect to their clinical outcomes. Methods: This is a retrospective comparative study in which patients who underwent surgery and one of the four types of IOL were implanted. Postoperative evaluation was recorded at one month, postoperatively. The monocular uncorrected distance visual acuity (UDVA) and corrected distance visual acuity (CDVA) (6 m), uncorrected intermediate visual acuity (UIVA) (60 cm), distance-corrected intermediate visual acuity (CIVA), uncorrected near visual acuity (UNVA) (40 cm), and corrected near visual acuity (CNVA) were assessed postoperatively on post operative day 30, for all four IOL groups. Defocus curve and contrast sensitivity were also compared. Results: With regards to UDVA and CDVA, P value was not statistically significant. (P = 0.534 and 0.421, respectively). EDOF group of IOLs had statistically significant better UIVA and CIVA than trifocal IOL group. (P < 0.001, 0.012, <0.001) and EDOF group had statistically significant worse P value pertaining to UNVA and CNVA (P < 0.001, 0.070, <0.001, 0.190). Pertaining to contrast sensitivity, EDOF group had better contrast sensitivity than Trifocal IOL group (P < 0.001). Conclusion: All four IOLs compared in this study had good comparable distant visual acuity. Near visual acuity was better with AT LISA and Eyecryl SERT trifocal IOL while intermediate vision was better with Eyhance and Eyecryl SERT EDOF IOL. Contrast sensitivity was better in EDOF IOLs than in both trifocal IOLs

ABSTRACT

Objective:To evaluate the effect of peripheral defocus soft contact lenses (PDSCLs), single-vision spectacles and single-vision contact lenses (SVCLs) on the progression of myopia in children and adolescents. Methods:A meta-analysis was conducted. To collect relevant studies on the myopia control effect of PDSCLs in children, English databases including PubMed, Medline, Embase and Cochrane library were searched with myopia, contact lens, children, adolescents, myopia progression, axial length, refractive error and relevant free English terms as key words.Chinese databases including CNKI, Wanfang and VIP were searched with corresponding Chinese phrases and relevant free Chinese terms as key words.Randomized controlled trials (RCTs) on the myopia control effect in children and adolescents, with PDSCLs wearer as experimental group and single-vision spectacles or SVCLs wearer as control group, were independently collected by two researchers.Quality of included studies was evaluated with the Cochrane tool to assess risk of bias for RCTs.Combined effects of change in refraction and axial length between experimental and control groups was calculated by weighted mean difference (WMD) and 95%confidence interval ( CI). The heterogeneity of included studies was evaluated by I2 statistic. The refraction and axial length of experimental and control groups were compared by Z test.Myopia control effect of different PDSCLs add powers was analyzed by subgroup analysis. The experimental data with add power ≤+ 2.00 D, low aberration and low depth of focus were assigned to low-medium add power subgroup, and the experimental data with add power >+ 2.00 D, high aberration and high depth of focus were assigned to high add power subgroup. Results:A total of 378 publications were retrieved. Finally, 10 high-quality RCTs and 14 groups of data were included in this meta-analysis.In these studies, 1 645 myopic children aged from 6 to 18 years were enrolled, including 808 cases in experimental group and 837 in control group. The follow-up ranged from 10 to 36 months.Among the 10 studies, there were two crossover trials without a washout period, so only the first intervention results were included.According to the meta-analysis, the change in refraction in experimental group was significantly less than that in control group (WMD=0.22 D, 95% CI: 0.15-0.30, Z=5.65; P<0.05). The change in axial elongation was significantly less in experimental group than control group (WMD=-0.10 mm, 95% CI: -0.12--0.09, Z=12.28; P<0.05). Subgroup analysis showed that the WMD of refraction change and axial elongation between experimental and control groups were 0.21 D (95% CI: 0.10-0.31) and-0.10 mm (95% CI: -0.13-0.08) in the low-medium add power subgroup, respectively, and were 0.26 D (95% CI: 0.13-0.38) and -0.13 mm (95% CI: -0.15--0.10) in the high add power subgroup, respectively. Conclusions:PDSCLs have better myopia control effect than single-vision spectacles and SVCLs in children and adolescents. When the add power is higher, PDSCLs can slow the progression of myopia more effectively.

ABSTRACT

@#Myopia is a common ocular disorder. In recent years, the incidence of myopia presents an increasing trend year by year. Patients with high myopia are at an increased risk for severe visual impairment. The economic and societal impacts of complication associated with myopia are enormous. Therefore, it is essential and imminent for implementing effective myopia control strategies. The results of the current study suggest that uncontrolled axial elongation produced by periphral hyperopic defocus could be one of the possible mechanisms for myopia development, multiple strategies especially optical defocus technology based on this are increasingly becoming part of the mainstream clinical practice in myopia management. This article reviews the principle of optical defocus on myopia control, experimental research on defocus myopia animals, and the latest clinical applications of different optical defocus technologies on myopia control, summarizes the clinical research results of myopia control using progressive addition spectacle lenses, peripheral defocus spectacle lenses, defocus incorporated multiple segments spectacle lenses, orthokeratology contact lenses and multifocal soft contact lenses. It is proposed to provide a new option for the treatment plan to delay the progression of myopia.

ABSTRACT

@#AIM: To evaluate the visual quality between the Tecnis Symfony extended depth of focus intraocular lens(IOL)and Zeiss trifocal IOL(At Lisa tri 839 MP)implantation.<p>METHODS: Forty-two patients(53 eyes)who underwent cataract phacomulsification with IOL implantation from Jan. 2019 to Dec. 2020 were randomly divided into group A and group B according to the type of implanted IOL. The group A with 22 patients(29 eyes)implanted the Tecnis Symfony extended depth of focus IOL, the group B with 20 patients(24 eyes)implanted Zeiss trifocal IOL. The uncorrected distance visual acuity(UDVA), uncorrected intermediate distance visual acuity(UIVA), uncorrected near distance visual acuity(UNVA), and best corrected distance visual acuity(BCVA)were measured after 3mo of surgery. The defocus curve, the quality of life and the occurrence of complications were also observed.<p>RESULTS: The visual acuity was significantly improved in both groups after 3mo of surgery. There were no significant differences in the UDVA(5m), UIVA(80cm)and BCVA(5m)between the two groups(<i>P</i>>0.05). In group B, the UIVA(60cm)and UNVA(40cm)were better than in group A(all <i>P</i><0.05). The defocus curve analysis revealed that the group A tended to be stable between +1.0D to -2.0D, and to form a platform, while the group B had double peaks(0D, -2.5D). The distance visual acuity at 0D were no significantly differences between the two groups(<i>P</i>>0.05), but the near visual acuity at -2.5D and -3.5D in group B were significantly higher than in group A(all <i>P</i><0.01). In both groups over 90% patients were spectacle independent. Patients in the two groups showed different degrees of halo, glare and other optical phenomena, but the overall satisfaction were higher. The scores of satisfactions for reading speed and night vision were significantly better in group A than in group B(all <i>P</i><0.05). <p>CONCLUSION: In both groups over 90% patients were spectacle independent, they can obtain good near, intermediate and far distance visual acuity. Symfony extended depth IOL has better continuous intermediate distance visual acuity, patients can also obtain higher satisfaction and have better night vision. The Zeiss trifocal IOL implantation is associated with significantly better near visual acuity, and it is more suitable for near vision.

ABSTRACT

PURPOSE: To determine the through-focus optical bench test performance of monofocal, bifocal, and extended depth-of-focus intraocular lenses (IOLs), and to measure their defocus curves.METHODS: A model eye was placed on an optical bench to test three different IOLs (TECNIS ZXR00, ZMB00, and ZCB00; Abbott Medical Optics, Santa Ana, CA, USA). The focus was changed by inserting trial lenses from +1.00 diopters to −4.00 diopters, in increments of +0.25 diopters. The 1951 United States Air Force Resolution chart was used to determine the quality of the images. The degree of similarity with reference images was given by the cross-correlation coefficient, and defocus curves were drawn and compared.RESULTS: Bifocal IOLs showed lower image quality with the addition of minus diopter trial lenses, but showed good image quality at near distance. Bifocal IOLs also showed a ‘double peak’ in their defocus curve. Monofocal IOLs showed a lower image quality and cross-correlation coefficient with addition of lower-diopter trial lenses. The extended depth of focus IOLs showed a single peak in their defocus curve, but had a wider range of diopters and better image quality than monofocal IOLs.CONCLUSIONS: Bifocal IOLs showed a double peak defocus curve, and extended depth of focus IOLs showed a wider diopter range and better image quality than monofocal IOLs.

@#AIM: To compare the visual quality and visual related quality of life in cataract patients after implantation of diffractive aspheric bifocal IOL and single focus aspheric IOL. <p>METHODS: Retrospective non-randomized study was implemented. In clinical application 110 patients(176 eyes)underwent phacoemulsification combined with IOL from October 2016 to December 2017. In details, 56 cases(82 eyes)with diffractive aspheric bifocal IOL(ZEISS AT LISE 809M)and 54 cases(94 eyes)with single focus aspheric IOL(ZEISS CTA 603P), respectively. Three months after surgeries, the main evaluation indices were successively collected, which included visual, the optical quality analysis system(OQAS)of the MTF cutoff, the Strehl ratio, the OSI, the 100% visual acuity(VA), the 20% VA and the 9% VA, as well as the visual function related quality of life scale(NEI-VFQ-25).<p>RESULTS: 1)VA: there was no significant difference in the corrected distance visual acuity(CDVA)between the two groups(<i>P</i>>0.05). However, there existed significant difference in the near vision(<i>P</i><0.05); 2)Objective quality visual analysis(OQAS): there was significant difference in the OSI. Regarding the visual acuity of simulated contrast sensitivity between two groups, the 20% VA and 9% VA expressed significant difference(<i>P</i><0.01); 3)NEI-VFQ-25 questionnaire: there were no significant differences in overall health, general vision, eye pain, distant vision, peripheral vision, social function, color vision, dependence and mental health between the two groups(<i>P</i>=0.88, 0.08, 0.60, 0.36, 0.80, 0.18, 0.41, 0.07, 0.95). But there was significant difference in near vision and social role limitation between the two groups(<i>P</i><0.05).<p>CONCLUSION: The 809M intraocular lenses and the 603P intraocular lenses can improve the optical quality of patients after the surgeries. The former one can provide better near vision, but the OSI was higher than the latter one, which may cause some visual interference. There was no significant difference in visual related quality of life after the surgeries generally.

ABSTRACT

@#AIM: To evaluate the interocular aberration root mean squares(RMS)characteristics among different severities of hyperopic anisometropic amblyopia.<p>METHODS: Sixty hyperopic anisometropic amblyopia were enrolled and divided into mild, moderate and severe groups. Each group was then divided into amblyopic eye(AE)and fellow eye(FE)subgroups according to the corrected visual acuity of each subject. All patients underwent wavefront analysis with iDesign© wavefront aberration system(AMO, USA)in mydriasis condition. Paired <i>t</i>-test was used to compare interocular differences of aberration parameters within groups. <p>RESULTS: Paired<i> t</i>-test showed that significant differences in defocus equivalent(DE), BCVA between AE and FE in mild, moderate and severe anisometropic amblyopia(all <i>P</i><0.01). No significant interocular difference was noticed in mild group except for forth-order aberration RMS(<i>P</i><0.01). And No significant interocular differences were noticed in moderate and severe group except for total aberration RMS(both <i>P</i><0.01). One-way ANOVA showed that significant interocular differences were noticed in third-order aberration RMS among the groups(<i>P</i><0.01). LSD showed that interocular difference of third-order aberration RMS in moderate group was significant lower than that in mild and moderate groups(both <i>P</i><0.05). <p>CONCLUSION: In mydriasis condition, mild anisometropic amblyopia showed higher forth-order aberration RMS in AE than that in FE, moderate and sever amblyopia showed higher total aberration RMS in AE than that in FE. Moderate anisometropic amblyopia manifested lower interocular differences of third-order aberrations RMS than mild and severe amblyopia, but no significant interocular differences observed between mild and severe amblyopia.

ABSTRACT

@#AIM: To study the efficacy of AMSPL on myopia control, and confirm the security of wearing AMSPL, through comparing the impacts on visual parameters between myopic children wearing spectacle lens designed to reduce peripheral hyperopic defocus(AMSPL)and myopic children wearing the standard design control lens(SPL).<p>METHODS: Totally 50 children aged 8 to 14 years wearing spectacle lens designed to reduce peripheral hyperopic defocus(AMSPL)were collected into the AMSPL group, and other 50 children in the same age, the same degree of myopia and the same glasses time wearing standard design control lens(SPL)were selected into normal control group randomly(SPL group). We reviewed their documents and exam all patients. The examination include intraocular pressure, refraction under cycloplegia, distant strabismus and near strabismus, AC/A ratio. <p>RESULTS: The children wearing spectacle lens designed to reduce peripheral hyperopic defocus(AMSPL)had lower feeling of comfort than SPL group, mainly in peripheral vision confused, but no difference between them 1mo later. The AMSPL group's average growth of refractive error is -0.62±0.50D, the SPL group's average growth of refractive error is -0.77±0.48D(<i>P</i>=0.072). In myopic children aged 8 to 10 years, the AMSPL group's average development of refractive error is -0.71±0.41D, lower than the SPL group that of -1.05±0.39D, the difference was significant(<i>t</i>=2.164, <i>P</i>=0.041). Between the two groups, there was no significant difference(<i>P</i>>0.05)in visual parameters of distant strabismus, near strabismus, AC/A ratio.<p>CONCLUSION: Wearing spectacle lens designed to reduce peripheral hyperopic defocus(AMSPL)can delay the progression of myopia to a certain extent, especially for myopic children aged 8 to 10 years. It suggests that wearing AMSPL has the same safety with SPL for myopic children.

ABSTRACT

@#AIM:To observe the different effect of controlling myopia by orthokeratopraxis with different width between defocus circle and the rim around the cornea.<p>METHODS: A retrospective case-control study. A total of 45 patients(85 eyes)who came to our hospital for orthokeratopraxis lens fitting during January 2014 to December 2015 were selected. The diameter of the orthokeratopraxis lens was 90%-95% of the diameter of the cornea. So the patients were divided into three groups according to the different diameter of the orthokeratopraxis lens. The diameter of the orthokeratopraxis lens for the patients whose corneal diameter was between 12.1 to 13.00mm was 11.0-11.2mm(Group A). The diameter of the orthokeratopraxis lens for the patients whose corneal diameter was between 11.1-12.00mm was 10.5-10.9mm(Group B). The diameter of the orthokeratopraxis lens for the patients whose corneal diameter was between 10.0-11.00mm was 10.0-10.4mm(Group C). The change of the spherical equivalent(SE)and axial length(AL)were analyzed after wearing the lens for 1a and 2a.<p>RESULTS: Before wearing lens, the age, SE(D)and AL(mm)was not statistically significant in the three groups(<i>P</i>>0.05). After wearing lens for 1a and 2a, the change of SE(D)and AL(mm)was not statistically significant in Group A and Group B(<i>P</i>>0.05)and the change was statistically significant in Group C(<i>P</i><0.05). The change of the SE and AL was also significantly different between Group A and Group C after wearing orthokeratopraxis lens for 2a(<i>P</i><0.05). A similar result was found between Group B and Group C(<i>P</i><0.05).<p>CONCLUSION: The effect of controlling myopia by orthokeratopraxis with different position of defocus circle on cornea is different. The position of defocus circle is more close to the corneal limbus, the effect is weaker.

ABSTRACT

Objective To observe c-Fos expression in visual cortex of infant rhesus monkeys with myopia induced by hyperopic defocus and preliminarily investigate the possibility of visual cortex participating in myopia. Methods Eight SPF grade healthy infant rhesus monkeys aged 20 to 30 days were randomly divided into hyperopic defocused group and control group,4 monkeys for each group. The monkeys in hyperopic defocused group wore -3 D spectacle lenses. The monkeys in control group wore 0 D lenses. The monkeys' refractive error, corneal topography, vitreous chamber depth were measured at the start of lens wear and at 2,4,6,8,12 weeks post-treatment. At 12 weeks post-treatment,the visual cortex tissues were removed for c-Fos protein measurement by immunohistochemistry and Western blot assays. The results were analyzed semiquantitatively to compare the differences of c-Fos expression between hyperopic defocused group and control group. The use and care of the animals complied with Regulations for the Administration of Affair Concerning Experimental Animals by State Science and Technology Commission. This study protocol was approved by Ethic Committee of Zhongshan Ophthalmic Center ( No. 2013-014). Results After 12 weeks'lens wear, the vitreous chamber elongation amplitude of hyperopic defocused group monkeys was more obvious than that of the control group ([0.93±0.24]mm vs. [0.72±0.09]mm;t=2.292,P=0.047). The decrease of hyperopic degrees of hyperopic defocused group monkeys was more obvious than that of the control group ([-3.23± 1.36]D vs. [-1.55±0.52]D;t=-3.273,P=0.006). The eyes of hyperopic defocused group monkeys appeared a remarkable myopic shift after treatment. The number of c-Fos immunoreactive neurons was less in the hyperopic defocused group than that in the control group, with a statistically significant difference between them ([1 843±191]/mm2vs. [2 296±503]/mm2;t=2.381,P=0.041). Western blot assay showed that the optical density of c-Fos protein in the hyperopic defocused group was significantly less than that in the control group (0.50±0.17 vs. 0.99± 0.22;t=-4.982,P<0.01). Conclusions Hyperopic defocus,as an abnormal visual stimulus, can induce the onset of myopia in infant rhesus monkeys and inhibit c-Fos expression in visual cortex. Visual cortex may participate in myopia induced by hyperopic defocus.
